How to Use the Formulary:

To use the formulary search tool:

1. Click the plan and click to proceed to drug search
2. Enter the drug name
3. Click the dosage and medication type you are searching for
4. Click the Add to List button

The list will display the drugs that are chosen and if the drugs are covered. If it is a covered drug the list will show if it is a generic drug or a brand drug as well as what drug tier it is on. If there are any restrictions (e.g., quantity limits), that will display as well.

What is a Formulary?

A formulary represents the prescription therapies believed to be a necessary part of a quality treatment program. Viva Medicare will generally cover the drugs listed in our formulary as long as the drug is medically necessary, the prescription is filled at a Viva Medicare network pharmacy, and other plan rules are followed. The formulary may change during the year. For more information on how to fill your prescriptions, please review your Evidence of Coverage.
