You will need your Medicare number that is on your Medicare card.
Medicaid Card
If applicable, you will need your Medicaid number that is on your Medicaid card.

Other Insurance Coverage Information

Social Security Card
If you plan to enroll in the Viva Medicare Extra Value or Extra Care plan, you will need your social security card.
Electronic Funds Transfer (EFT) form
If you want your premium to be automatically deducted from your bank account each month.
