Vision Benefits
VIVA MEDICARE offers an allowance toward prescription eyewear (glasses, contact lenses, frames, and upgrades) and contact lens fittings. You can use the eyewear provider of your choice.
Plus, get one pair of eyeglasses or contact lenses for a $0 copay after cataract surgery. You pay any amount over the Medicare allowed amount.
Members who pay out-of-pocket can submit a request for reimbursement to Viva Medicare by filling out a copy of the Reimbursement Claim Form below. Or, call Viva Medicare Member Services at 1-800-633-1542 (TTY: 711).
