2026 Plan Choices

Viva UAB is a limited network plan, which means your health insurance is limited to using UAB Health System, Medical West, Cooper Green, and UAB St. Vincent’s for primary care, OB/GYN, and most other services. You will have lower cost sharing amounts on this plan.

UAB Network

Only doctors and hospitals affiliated with UAB Health System, Cooper Green, Medical West, and UAB St. Vincent’s are in your network.

Viva Choice provides you access to the full Viva Health network of doctors and hospitals. Your out of pocket costs are the same regardless of where you receive care.

Here's how it works:

What You Pay

You pay the first $1,650 (single coverage) or $3,300 (aggregate amount per family) of your care.

Out of Pocket

You then pay 10% for most of your care until your out of pocket costs reach $3,300 (single coverage) or $7,100 (family coverage) for the plan year.

What We Pay

The plan pays 100% of the cost once you’ve reached the annual out-of-pocket maximum.
